HealthyCircles will leverage Surescripts' Medication History
Acute service to provide hospitals and their extended acute care
facilities a solution to address medication reconciliation,

to the Surescripts network to access Surescripts' Medication
History Acute service. The combination of HealthyCircles' Care
Coordination capabilities and Surescripts' medication history
service will offer hospitals and extended acute-care facilities a
powerful toolset to manage the discharge process, coordinate
transitions of care and reduce 30-day readmission rates.
