Request An Address Change
Prescribers:
If your address, phone or fax number has changed, or is not correct in the Surescripts directory, please contact your e-prescribing/EMR vendor to ensure this information is updated.
Note: A prescriber's directory listing should mirror the
information on their paper prescription pad. The fax number listed
should be the number where prescription renewal requests should be
communicated for the prescriber.
Pharmacies:
If your address, phone or fax number has changed, or is not correct in the Surescripts directory, please contact your pharmacy management system vendor to ensure this information is updated.
Note: If there has been a change in NCPDP/NABP ID please work with your vendor to deactivate the old NCPDP/NABP listing and enroll the pharmacy under the new NCPDP/NABP ID with proper demographic information.
