HealthyCircles Connects to the Surescripts Network
to Reduce Hospital Readmissions


HealthyCircles will leverage Surescripts' Medication History Acute service to provide hospitals and their extended acute care facilities a solution to address medication reconciliation, post-discharge care coordination and reducing hospital readmissions.

Today, HealthyCircles™(www.healthycircles.com), an integrated suite of online Professional and Consumer portals, announced it will connect to the Surescripts network to access Surescripts' Medication History Acute service. The combination of HealthyCircles' Care Coordination capabilities and Surescripts' medication history service will offer hospitals and extended acute-care facilities a powerful toolset to manage the discharge process, coordinate transitions of care and reduce 30-day readmission rates.

Surescripts' Medication History Acute service allows prescribers and authorized staff in acute-care settings to query and receive aggregated details for up to a year's worth of patient medication history from payer and pharmacy records, representing over 240 million patients. The HealthyCircles Transition of Care service is designed to support the needs of acute-care facilities, hospital emergency departments and critical access hospitals, including those facilities that are rural or small.
